Mission Survey Form

Please complete this survey, then click on the "Send" button to send it.

Note from the webservant: This survey has not been working recently, but we believe that the problems have been corrected. If you have submitted a survey, we did not receive your input. Please accept my apologies. If you would resubmit your information, we would be grateful.

Please note: The purpose of this information is to create a data base which will be used to identify what churches in the St. Louis Metro/Metro East area are doing in cross-cultural missions and obtain more accurate date on people groups in the area, especially those without a viable church in their culture. General information will be used in tables, graphs, prayer tools, or other strategy information to assist ministries for outreach. Contacts information or church specific can be used to facilitate partnerships, share resources or notification for specific events, but will be released only if you indicate your willingness to do so.
